A free yearbook ads and dedications form
A yearbook dedication is a photo and a short message from a family, printed in every copy of the yearbook to celebrate a child, most often a graduating 5th grader. Collect them with a simple Google Form, set your own prices and deadline, and the money goes to your PTA or PTO.
Nothing here depends on your yearbook vendor. You collect the submissions and lay them out on your dedication pages like any other page.

What is a yearbook ad or dedication?
A short message printed in the yearbook to celebrate a student, thank a teacher or send a shoutout to a graduating child, usually with a photo.
It’s different from a personalized page. A personalized or custom page appears only in one family’s copy. A dedication appears in every copy, so a child sees their family’s message next to their friends’. Many families do both, because they do different jobs.
Who can take part?
Participation is optional. Schools usually open it to:
- parents and guardians
- grandparents and relatives
- approved community members or local businesses, if your school chooses
Your school decides who may participate and how submissions are offered.
How much should we charge for a yearbook dedication?
You set the prices. These are suggested starting points for an elementary school, to adjust to your community:

Offering a lower-priced option makes it easier for more families to take part. Prices should feel accessible while still supporting your fundraising goals.
How do I set up the form, step by step?
- Copy the form into your Google account and rename it with your school and year.
- Define the page sizes you’ll offer, and add their prices to the form.
- Set word or character limits for each size, so every message fits its space.
- Specify image requirements: file type and quality. Ask for the largest original photo, not a screenshot.
- Decide how families pay. Take payment however your PTA already does, and ask for a payment reference on the form.
- Add your deadline to the top of the form. Set it before your photo deadline, so the pages can be laid out with the rest of the book.
- Reserve the pages on your ladder. Dedications live in the back matter, and they’re the pages people forget until two days before the deadline. Plan the pages →
- Send the announcement (below) and at least three reminders before the deadline.
- Review every submission before it goes in the book.
What are the content rules?
To keep the yearbook inclusive and school-appropriate:
- Messages should be positive, respectful and student-centered.
- Photos must be family-approved and appropriate for school publication.
- All submissions are reviewed by the school or yearbook committee.
